If parents wish for their child to take up a ’30 hour provision’ place, they must first complete the government application process to ensure their eligibility and provide the school with the following information. (It is possible to apply for an eligibility code 16 weeks before the term you want your child to start or is eligible to start);

- Full name of parent completing the application
- DOB of parent completing the application
- National Insurance Number of parent completing the application
- Telephone number for parent completing the application
